If you have recently lived in Australia:

For entry into the New Zealand Diploma in Marine Engineering (Level 6) or the New Zealand Diploma in Nautical Science (Level 6), applicants need to complete this Maritime New Zealand medical declaration form.

For entry into the Diploma in Foreign-going Master or Chief (Level 7) Chief Mate/Master or Chief Engineering Foreign-Going,?applicants need to complete this Maritime New Zealand medical declaration form.

As part of the New Zealand Diploma in Nautical Science, ākonga must study STCW Proficiency in Survival Craft (PISC). The pre-requisite for this paper is proof of 12 months of sea service. This is the only paper in the programme where completion of the Certificate of Sea Service is required.
